The Australian Government is introducing a new and compassionate initiative within the Support at Home program – the End-of-Life Pathway Aged Care. This change recognises the importance of helping older Australians remain in their homes during their final stage of life, surrounded by loved ones and familiar comforts.
What Is the End-of-Life Pathway?
This pathway provides access to up to $25,000 in care and support services over a 12-week period for older people diagnosed as having less than three months to live. The funding is designed to make home-based end-of-life care more accessible, ensuring individuals receive the dignity, comfort, and care they deserve.
Who Is Eligible?
To access the End-of-Life Pathway, a GP or nurse practitioner must certify that the person is likely to have less than three months to live. The health professional will complete a short, secure form which triggers rapid approval through My Aged Care. If the person is already receiving aged care services, the transition is seamless.

What Services Can Be Funded?
The funding covers a wide range of services, including:
– Personal care and assistance with daily activities
– In-home nursing and health support
– Mobility aids and assistive equipment
– Home modifications to ensure safety and comfort
– End-of-life care planning and emotional support
What If More Time Is Needed?
The initial funding lasts for 12 weeks, but can be extended to 16 weeks. If additional care is still needed, another assessment may be carried out.
